On Dec. 23, 2016, a 46-year-old male worker at Autoneum North America in suburban Toledo, Ohio was guiding scrap pieces into an unguarded shredding machine.

The worker’s arm became caught in the machine’s circular drum which shreds fabric fibers for later use. As a result, the man’s right hand, wrist and part of his forearm were amputated.

OSHA began an investigation immediately after the incident and discovered the company failed to equip the machine with adequate safety guards.
